Radioactive Material Loss/Theft Procedure
The initial suspicion of loss of theft of radiation material
requires the immediate notification of the
Radiation Safety Officer (RSO) by email or voice mail. The
information needed is:
- Radioisotope
- Chemical and physical form
- Quantity (activity)
- Location from which it is missing
- Principal Investigator's name
- Person reporting the loss/theft
- Date and time it was discovered to be missing
The RSO will determine the extent of hazard presented by the
release of the material. Dependent upon the degree
of the threat to public health, the DOHS
Radiologic Health Branch, VP for
Administration, VP for Academics, University
Police and/or the Radiation Safety
Committee will be notified. At a minimum, a report must be placed in the
Radiation Safety files for all losses or thefts (as
noted below)
Any loss of suspected theft must be thoroughly documented.
The report must become a part of the RSC Quarterly
Report. A copy will be sent to the DOH,
Radiologic Health Branch upon their
request.
